In many countries all over the world there is an acute lack of physicians and medical staff in the hospitals, many times the physicians are overwhelmed by the duties and tasks that they must accomplish. Sometimes when the physician is visiting the patients he is accompanied by other medical staff including nurses and resident students, after the physician is consulting the patient observation sheet and he is inspecting the current health status of the patient, the physician makes observations which are noted by a nurse or a resident student, which after the visit is writing the notes into the patients observation sheets. Many times, some information is omitted or deformed due to the person which is noting the observations. The paper presents a system which was initially designed to manage the observation sheet and allow the recording of the physician observations to avoid errors in recording the information in the observation sheet. The system allows also to train the resident students based on case studies but in the same time protects the patient privacy.
