Efavirenz is a novel non-nuclear acid reverse transcriptase inhibitor (NNRTI) of human immunodeficiency virus (HIV) that is well tolerated and often combines two nucleotide reverse transcriptase inhibitors (NRTIs) to be applied to the treatment of the first-line anti-infection. The metabolism of efavirenz is associated with a variety of cytochrome oxidases, and is mainly metabolized by the liver microsomal cytochrome P450 isoenzyme, which can induce CYP450 to perform its own metabolism. Substrates, inducers and inhibitors of these enzymes may affect the blood concentration of efavirenz. Ketamine is mainly metabolized by cytochrome isozymes, when especially combined with ritonavir, nelfinavir and efavirenz, their inhibitory activity will increase the blood concentration of ketamine, and cause the heart rate accelerating and the blood pressure rising or respiratory depression. In this study, we discuss the effects of anesthetic drugs on antiviral drugs from the perspective of pharmacokinetics, and explore the perioperative adjustment of drug regimens to improve treatment.
